Geriatric Patient Care

The Division of Geriatric Medicine and Palliative Care works with a singularly diverse population because it has active clinical and educational programs at multiple institutions. Our faculty has broad clinical expertise and strong institutional involvement in geriatrics and/or palliative care. Several Geriatrics faculty members have a strong interest in healthy aging, while others work with complex patients. Several faculty members are board certified both in Geriatrics and in Hospice and Palliative Medicine.

Clinical activities include primary care of the older adults, inpatient geriatric consultation, home and community based care, and sub-acute and long-term care in nursing homes. All of our Geriatric primary care sites, in the VA Hospital, Bellevue Hospital and Tisch Hospital, are involved in medical home and care-delivery redesign activities. The Geriatric Clinic at the VA Hospital on East 23rd Street in Manhattan (part of the Veterans Affairs New York Harbor Healthcare System) is grounded in systems based approach to care, and has an established medical home for primary care and geriatrics. The Tisch Hospital Geriatrics Clinic is part of a NCQA certified medical home practice.

Palliative Care

Palliative Care involves the relief of the pain, symptoms and stresses of serious illness. Our Palliative Care team is dedicated to improving the quality of life for people with life-threatening or terminal illnesses — especially with conditions that are progressive and limit function and quality of life.
